Entries Open
Entry opens in May 2019. You'll get the opportunity to tell us your story, what you've done and how it has had a positive impact on safety at your workplace. You can start now; starting thinking about your safety story and what you think is most important about your initiative or achievement.
You can enter yourself, a co-worker or an organisation to be rewarded for commitment to work, health and safety.
Choose the category that best suits your business and your solution. You can enter in more than one category.
Collect up to four images that represent your entry, as well as your company logo. Accepted formats: jpeg, gif or eps. You can also include links to YouTube or Vimeo clips.
Entries are submitted online. Be sure to address all criteria!
Entries that demonstrate a significant achievement in work health and safety and/or return to work will go through to site visit.
We will contact you if you progress to site visit. You may need to provide additional information at this stage.
Entries that go through to site visit are assessed against set criteria.
SafeWork NSW experts will assess the site visit results to select the finalists.
All finalists are invited to the prestigious ceremony and dinner where the winners will be announced.

Find a few images that best represent your entry. The images can show either examples of the changes the entry has made in your workplace or the things people have done to make improvements. (The images must be in jpg or eps format. Images embedded in Word or PDF documents will not be accepted.)
Prepare any video supporting footage. You will be able to add in a link to your YouTube or Vimeo video in the uploads section of the form.
Think about your entry. What did you do to improve work health and safety or return to work. How did you do it? What problems did it solve?
