Step-by-step guide to entering the Awards:

1. Read through the categories and decide which one your safety initiative is most suited to. You may decide to enter more than one category.
2. Complete an entry form for each category entered.
3. All entries must be accompanied by supporting documentation prepared by the applicant.
4. To have your entry considered by the judges you must address every criterion. Keep your responses brief and to the point – approximately one page per criterion should be sufficient.
5. As part of your entry please include with your entry form:
   • A summary of your initiative and company profile and complete on the entry form
     • Each summary should be between 200 and 300 words. The information will be used on the WorkCover website if you are a finalist or winner to promote your initiative or innovation.
   • Supporting information that you think might help the judges to better understand your initiative.
     • For example, a workplace safety manual you’ve developed or photographs of the initiative(s).
   • Photos as prints or as digital photos
     • If you choose to supply photos digitally, there must be four separate copies either on CD ROM (in high resolution 300dpi format) or upload using the online entry form
   • Four copies of your hardcopy submission (all unbound A4 format), unless using the online entry form
   • Your company logo provided as a high-resolution(eps file) on CD ROM or can be sent via email to swa@workcover.nsw.gov.au
6. Refer to the checklist at the end of your application form to ensure you have included everything.
